Will this actually fit my Karl Storz light source?
Yes — for all standard Karl Storz light sources using the 495NE / 495ND fitting (which is the current-generation Storz standard: Xenon Nova, Xenon 300, Power LED). Autoclave-safe?

How can I tell when a cable needs replacement?
